If you do not have a Homelink Parent Portal account and would like to participate in the Online Data Confirmation process, please contact the Bridge Office Manager.

The Bridge Program is for individuals ages 18-22 years old with disabilities who have finished high school.
The program is based on the federal and state mandates to provide transition to adult living, and embodies California’s core messages for transition:
- Student-focused planning
- Student development
- Interdisciplinary and interagency collaboration
- Family involvement
The goal of the Bridge Program is to provide individuals with the knowledge and skills they need in order to make a successful transition to adult life.
